Balanced by Bristlecone Consultation Registration

If you think Balanced by Bristlecone is something that you would like explore further, we ask that you set-up a one time 30 minute consultation with Sadie at Bristlecone. This time will be used to make sure that you feel this program is well aligned with your needs before any commitment is made. You should be prepared to discuss your goals and desires.

Consultation cost is $35 and must be paid prior to meeting with Sadie. All consultations are held on Thursday’s from 9AM to 12PM by appointment only at Bristlecone, 67 W Center St, Cedar City UT.

In consideration of my use of the facilities, equipment, and all instructor lead programs provided by The Bristlecone Group, LLC (hereinafter referred to as "Bristlecone"), I expressly agree and contract, on behalf of myself, my heirs, executors, administrators, successors and assigns, that Bristlecone and its owners, insurers, employees, and all other representatives, shall not be liable for any damages arising from personal injuries sustained by me, on, or about the premises, or as a result of the use of the equipment, facilities, and instructor leads programs, regardless of whether such injuries results, in whole or in part, from the negligence of Bristlecone.

By the execution of this agreement, I accept and assume full responsibility for any and all injuries, damages (both economic and non-economic), and losses of any type, which may occur to me, and I hereby fully and forever release and discharge Bristlecone, its owners, insurers, employees, and all other representatives, from any and all claims, demands, damages, rights of action, or causes of action, present or future, whether the same be known or unknown, anticipated, or unanticipated, resulting from or arising out the use of said equipment, facilities, and instructor lead programs.

I expressly agree to indemnify and hold Bristlcone harmless against any and all claims, demands, damages, rights of action, or causes of action, of any person or entity, that may arise from injuries or damages sustained by me or my guest.

I understand and agree that Bristlecone is not responsible for property that is lost, stolen, or damaged while in, on, or about the premises.
